By utilizing the Checkout Window, you are agreeing to the General A&D Facilities Policies, as well as the Checkout Window Agreement Terms listed below:

Checkout Window Agreement Terms

• I understand that I may not use anyone else’s access, nor can I authorize anyone to use my access
• I understand that failure to comply with any policy may result in the loss of Facilities Access and privileges
• I will present my i-card for all transactions at the Checkout Window, stay present for all transactions, and I will not leave any items unattended
• I will be assessed a LATE CHARGE per day for each item not returned on time, or not in a condition to be checked out to another patron
• I agree that I am fully and financially liable for the repair or current replacement value of any equipment damaged or not returned, items that are returned in an unusable state, and items that are late by more than 30 days, or by the close of the Checkout Window at the end of the semester
• I will examine each of the items I receive before leaving the Checkout Window and I will make sure they are in good operating condition; I understand that I become liable for any unreported damage or missing parts
• I am responsible for knowing when my loan is due back
• I understand that there are no renewals. I must return my item(s) at the end of my checkout
• I understand that Studios must be returned to a usable state, vacated, and room keys must be returned 15 minutes before closing time, or I am subject to late fines. If I bring models into the studio, they cannot use or handle any equipment, and they must be accompanied by me at all times. I am responsible for the behavior and safety of any individuals that I permit into the studio
• I understand that the studio environment can present electrical and other hazards THAT CAN CAUSE SEVERE INJURY (high voltage strobes, hot lights, falling objects). I will not use any equipment that I have not been trained to use by my instructor
• I agree that I will use each item properly and only in the manner for which it was designed and I have received adequate training and instruction on how to operate the equipment or resource before attempting to use it
• I hold the University of Illinois, the School of Art & Design and its employees not liable from any and all claims arising from my use or possession of items loaned to me, and not liable for lost time or the unavailability of equipment or processes for any reason
